Waiver and Release of Liability Agreement
Please read and acknowledge the following terms before participating in the Christy’s Foundation Volunteer Program:

Acknowledgment of Risks: Participation in the Volunteer Program involves potential risks, including personal injury, illness, or death, and risks associated with the transmission of COVID-19.
Assumption of Risks: You willingly assume all risks, known and unknown, even those arising from the negligence of the program's organizers and other related parties (the "Releases").
Compliance with Terms: You agree to adhere to the program's terms and conditions and report any unusual hazards immediately.
Release of Liability: You release the Christys Foundation and associated entities from any claims or liabilities resulting from your participation, including injury, illness, death, or property damage.
Binding Agreement: This agreement is binding on your heirs, assigns, and legal representatives, releasing the Release from liabilities, including those resulting from negligence.
Voluntary Consent: You understand the terms and sign this agreement voluntarily, relinquishing certain legal rights without any inducement.

Photo & Video Waiver/Consent: The undersigned gives permission to the Christy’s Foundation to use the undersigned’s picture/video or other likeness or a picture/video or other likeness of the undersigned’s children for any purpose, including, but not limited to, general marketing and publicity, social media presence and/or campaign or other promotional materials.
